The Contact Us Online form, allows you to request urgent and routine appointments online and also make admin requests.

This service hours are operational between 8:00am until 6:30pm this is to replace the phone call system.

Please do try use this online service from now on. There are many benefits as stated below

• Just like writing an email - 5 minutes to fill in your symptoms and availability 
• The link is open at 8am everyday Monday to Friday 
• We'll get back to you via SMS within 2 hours or 2 days, depending on urgency.

If you still call us or walk in to request appointments, our team will fill the form with you and follow the same process above. This is to maintain the fairness to all patients.

Patients who require to be seen urgently can call 01604 716500 between 08.00am and 6.30pm by selecting option 1 and speaking to a receptionist who will care navigate you to the service that is most approriate to your needs.  They will need a brief description of your symptoms for the duty team who will call you back; this is called triage.

The Triage Team will prioritise work and will contact you no later than 2 working days.  If you are unable to wait and feel it is urgent then to ring 111 for advice.

Please make sure you are available for a call back if you need urgent attention.  

We will contact everyone who feels that they have an urgent problem on the same day.
