Online Access
There are a number of ways you can now access your NHS Records:

SystmOnline is the software we use to store your medical information. Patients are able to access it themselves via a secure account which gives access to:

- Order your repeat prescriptions
- Make an appointment
- Cancel an appointment
- Change your contact details
- View your medical record
Register for Online Services
We will ask you to complete the Patient Request for Online Access (form located below) which includes the requirement to understand and agree to the Practice terms in relation to online accessd. Your GP record includes information like any conditions or allergies you have any medication you are taking.
Once we have your request to register for the Online service your login information (username and password) will be sent to you securely via SMS or email. If you wish to collect a printed login you will need to attend in person bringing a form of photo identification with you such as a passport or work ID. Each patient requires their own login and has their own account.
When you log in for the first time you will be required to change your password to something easily memorable to you.
When you successfully log in you will have immediate access to appointment booking and the ability to request a repeat prescription. If you would like to see more of your record you can request access to your Summary Record as well as your Full Clinical Record (which has superseded the now obselete Detailed Coded Record). Click on the Add button and your request will be processed within three weeks.
Children must be aged 11 or over to have their own account. Parents wishing to have online access for their children will need to complete proxy access forms (see below).
Proxy Access to Medical Records
If you book appointments or request medication for a relative or someone you care for, it is possible to have online access, known as proxy access, to do this. A Consent to Proxy Access form must be signed by the person (this includes children over the age of 11). Proxy access given to parents is automatically halted when the child turns 14 and must be re-requested or the child can have their own online account.
The person being given proxy access does not have to be registered with us.
To request proxy access please complete the form available here and hand in to the surgery where it will be processed within three weeks.

NHS App
To use the NHS App you must be aged 13 or over and registered with a GP surgery in England. Find out more about who can use the NHS App at www.nhs.uk – Who can use the NHS App.
Use the NHS App to:
- get your NHS COVID Pass – view and download your COVID Pass for places in England and Wales using this service, or for travel abroad
- get advice about coronavirus – get information about coronavirus and find out what to do if you think you have it
- order repeat prescriptions – see your available medicines, request a new repeat prescription and choose a pharmacy for your prescriptions to be sent to
- book appointments – search for, book and cancel appointments at your GP surgery, and see details of your upcoming and past appointments
- get health advice – search trusted NHS information and advice on hundreds of conditions and treatments. You can also answer questions to get instant advice or medical help near you
- view your health record – securely access your GP health record, to see information like your allergies and your current and past medicines. If your GP has given you access to your detailed medical record, you can also see information like test results and details of your consultations
- register your organ donation decision – choose to donate some or all of your organs and check your registered decision
- find out how the NHS uses your data – choose if data from your health records is shared for research and planning
- view your NHS number – find out what your NHS number is (www.nhs.uk – What is an NHS number?)
Sharing Your Medical Record
Increasingly, patient medical data is shared e.g. between GP surgeries and District Nursing, in order to give clinicians access to the most up to date information when attending patients.
The systems we operate require that any sharing of medical information is consented to by patients beforehand. Patients must consent to sharing of the data held by a health provider out to other health providers and must also consent to which of the other providers can access their data.
e.g. it may be necessary to share data held in GP practices with district nurses but the local podiatry department would not need to see it to undertake their work. In this case, patients would allow the surgery to share their data, they would allow the district nurses to access it but they would not allow access by the podiatry department. In this way access to patient data is under patients' control and can be shared on a 'need to know' basis.
