Medical matters and official complaints cannot be dealt with via this form. If you have a query regarding a medical matter please telephone reception to make an appointment to see the appropriate person.

Want to make a complaint?

We will always do our best to look after you as well as we can but we accept that there are going to be times when we do not meet your expectations and for whatever reason you feel that you need to make a complaint.

 

Complaints are an essential tool in maintaining and improving standards.

 

You can make a complaint in writing to:

By Telephone by asking to speak to: The Practice Manager or Deputy Practice Manager.

 

After you have made your complaint we will acknowledge it as soon as possible and then investigate it for you.

 

Complaints are handled in two stages.  Firstly they are investigated locally and the results shall be given to you for your comments.  If you are not happy and you wish to take your complaint further then you should contact:
